Authorisation for data processing
This is the text of the authorisation you accept when ticking the consent checkbox on any of our forms.
Data controller
Pending legal validationThe full identification of the data controller — registered name, tax identification number, address and channel for data subject requests — is pending legal validation and is not published until the clinic confirms it.
What you authorise
By ticking the box you give prior, express and informed authorisation for the processing of the personal data you provide, for the purposes described in the data processing policy.
This authorisation is voluntary. Without it we cannot contact you to respond to your request, so it is essential in order to process it.
Separate authorisation for communications
Authorisation to receive commercial communications is a SEPARATE, optional checkbox.
Not ticking it has no effect whatsoever on how your request is handled. You may withdraw it at any time.
Separate authorisation for clinical images
Publishing clinical photographs requires a DIFFERENT, specific authorisation, given in writing at the practice and never through this website.
Consent to receive treatment is NOT equivalent to consent to publish images. They are separate acts.
That specific authorisation details which images are published, in which media, for how long, and whether they are published anonymised or with identification. It may be withdrawn at any time, in which case the images are removed from the site.
How to withdraw authorisation
Pending legal validationYou may withdraw any of these authorisations by contacting the data subject support channel.
The email address and formal procedure are pending legal validation; in the meantime you may use the clinic’s general contact email.
